Creating / Completing an IDF Form
Directions:
Classroom teachers and specialists will create IDFs (literacy, numeracy, or behavioral) during SRBI meetings, enter data in the IDFs, and update information as needed.
*Prior to beginning a Tier II intervention, teachers need to check the Exit folder to see if the student received prior intervention(s). If they did, a new form does NOT need to be created. The IDF form in the Exit folder can be used to continue the current year’s intervention, but should be moved to the Active Folder.*
How to Create an IDF Form:
Click on the appropriate (elementary or secondary) IDF workbook below.
Rename it: student’s graduating year, first four letters of last name, first four letters of first name. Example: 27Jonejohn .
Select “file” and “move to” the student’s graduating year in the Student Intervention Documentation Forms.
Select “Bethel Public Schools SRBI folder”
Select “Student Intervention Documentation Forms”
Select “Class of ___ Intervention”
Select “Class of ___ - Active Intervention”
How to collaboratively complete the IDF Form:
Fill in the “Student Overview” with all of the available information you have on the student.
Select the appropriate tab corresponding with the intervention (Literacy/Numeracy/Behavior).
As a collaborative team complete sheet with baseline data and action plan for intervention.
Choose Format/text wrapping/wrap before entering information onto the IDF form. When typing in an active box, the box will expand while info is added. When you leave the box, it will revert to its original size. Also, use the vertical alignment button in the menu to bring the data to the top of the box.
4. IDFs will be shared with appropriate staff members.
Maintaining IDF Forms
Active Intervention Folder
● All IDFs must be kept current in the Active Folder
● When a student overview form indicates that a student has exited ALL interventions (literacy, numeracy, and behavior) the student’s IDF must be moved to the Exited Intervention Folder
Exited Intervention Folder
● All IDFs of students who been exited from ALL interventions will be housed here
● If a student’s IDF is in the Exited Folder and needs a new intervention, move their IDF form back into the Active Folder
● Place students who have moved out of district into this folder
End of Year Watch List
● At the end of year SRBI meeting, teachers will create a watch list of students that have not received intervention but have areas of concern
● Add the student to the list and add notes that will be helpful for the next year’s teacher
Closing Out The School Year
End of year checklist for SRBI team:
Review end of year data
Identify students that will receive Tier 2 or 3 intervention for the next school year
Create watch lists for next year
Make sure all IDFs are updated and labeled
Move exited/moved students to “Exit” Folder
Share folders with next year’s designated personnel
