1. Complete Volunteer Release Form.

2. Send Scan or Print of form to Coach Guha.

3. Once office has signed the form they will notify Coach Guha, he will notify you to complete online portal for Background Check (no cost no walk in!) Please wait to be notified by email before completing the online form.

4. Wait 2-4 weeks, receive email regarding completed background check.
