EVERY Avalon sport requires at least two items.

  1. The first is a "general" registration form for that season's sport. The example here is the 2021-2022 fall sports general sports sign up. This form puts you on the mailing list for that sport's important announcements.

  1. The second is the Avalon permission form. This form gives your student permission to participate in that sport, and holds Avalon legally harmless for injuries sustained during that sport. It also includes the fee schedule for the indicated sport. Please submit the fee along with this form. This form can be filled out via paper and faxed, mailed, or handed in in-person. You can also fill the form out online and "sign" the form by placing your name on the signature line, but the form MUST be emailed from the parent/guardian email address on file at Avalon. This is the example cross country running permission form for Fall 2021.

2. Cross Country Running Permission Form

If you are submitting paperwork for a varsity sport (Volleyball, Basketball, Nordic Skiing, Cross Country Running Varsity), continue to number three.

If you are submitting paperwork for a middle school sport (MS run club), non-mshsl sport (Ultimate Frisbee), or are submitting your second MSHSL sport in an academic year (Nordic Skiing and you previously joined Cross Country Running Varsity that year), please go to number 5.


  1. For varsity-level sports, additional forms are required by the Minnesota State High School League (MSHSL). The first is a sports physical form which is good for 3 year. It also INCLUDES the health questionnaire for that year. Be sure to fill out the student/parent/guardian portions of that form before submitting it to your doctor to complete during the physical exam. If you already had a physical exam this year before recieving this form, you may still be able to send this form to your doctor and have them fill it out. Some offices also send a seperate, distinct form directly to the school. If that is the case, be sure to fill out the annual health questionnaire.

3. Sports Physical Form (Good for 3 years)* (Varsity Only-must be printed and brought to physician)


  1. Another varsity-only form is the Eligibility Statement and Annual Health questionnaire, both included in the same form. The eligibility statement is very long and detailed and legal, but the checkboxes are fairly straightforward. Of note, if you cannot check all of the eligibility options on the first page, that does NOT deem you ineligible for sports. Please email chris@avalonschool.org for any eligibility questions. Also, please be sure to watch the concussion video and understand the concussion resources. The health questionnaire is included in this form, but you may have already completed it if you submitted a new sports physical form this year.

At the end of the eligibility statement there is a health questionnaire. This is the same one that is at the end of the sports physical. If you are submitting a new sports physical and you fill out the health questionnaire there, you do not need to do it again on the eligibility statement. However, if you already have a sports physical on file, you will need to submit a new health questionnaire along with the eligibility statement each year.


5. Online payment (can also mail or bring money in)

This is a link to a paypal account at Avalon. Any money that goes there this year we know is for sports (not field trips). I know it's confusing.

This is only for skeirs if they need equipment!

7. Transfer Eligibility Form

***Transfer Students: If you transferred to Avalon in the past 365 days, please fill out this form: Transfer Eligibility Form

All transfer students are eligible to participate in sports at Avalon, however there is a chance that they cannot be a part of the "varsity" team, which may mean they would race with JV players, play on a JV team, and not be eligible for conference, section, or state competitions for a period of one year from transfer.

MSHSL has decided that in an attempt to reduce school transfers and decrease competitive imbalance by "stacking teams," they have created a one-year varsity level ineligibility after a transfer, with some exceptions. If you move districts, transfer due to a court order or bullying/harassment, transfer from a licensed mental health or substance treatment program, are new to 9th or 7th grade for the first time, or have other extenuating circumstances you may be eligible to play at the varsity level.
