STEP 3: Prepare and Submit Your CPT Application

PART 1: PREPARE YOUR DOCUMENTS
You will need to have the following documents ready to upload:
Copy of the Original Offer Letter.
The ISSC CPT Employer Information Sheet from ISSC CPT webpage, click here.
The form is located under "How to Apply"
When saving this PDF document, please "Print to PDF"
This form will need an employer supervisor signature
Job description: This is not a separate form. The Adobe Sign and the CPT Information Sheet both have fields for this information.
Educational Objective (EO): You will need to write a short but detailed paragraph on how the internship is applicable to your program. To avoid any issues with Educational Objective being cut off it's recommended to do on a separate document and attach it to the AdobeSign form.
Review instructions on how to write an educational objective. If this needs to be rewritten you will need to start the process over again.
If you are doing another internship with the same company, your educational objective must be different and needs to highlight how the project work you will be doing is different from the work you did during the previous internship.
All requested documents must be uploaded within the AdobeSign form. Incomplete applications will not be processed. If you do not allow enough time for processing your application will be denied and you will be asked to adjust your start date.
Please check each box to make sure you have all the necessary documents. This will ensure that all required documents are included to avoid delays in the application process!
Once you have ensured that all your necessary documents are ready, review the comprehensive instructions for Adobe Sign outlined in the slides below.
PART 2: COMPLETE AND SUBMIT CPT APPLICATION
If you are a Thesis or Ph.D. student your faculty chair must review & approve your applicaton
Important notes:
If your employer is unable to sign the Adobe form, please contact advising for assistance.
Once signed by all (student, SCAI Advising, employer, and faculty chair if thesis/PhD), a confirmation email will be sent to all parties.
There is no need to send follow-up emails alerting us that you have submitted an internship request.
Requests that are submitted incorrectly or missing any documents will be denied and the process will restart, including the processing time.
Do not send follow-up emails inquiring about how long it will take to process your application. Processing times are posted above. If you did not allow enough time for processing you will be asked to adjust your start date.
