All medications administered at school must be authorized by the parent/guardian. In addition, some prescription medications require a signature from the prescribing medical provider. The school nurse will review all medication requests for appropriateness, including those for a field trip or overnight school travel. Please access the medication form link to find the appropriate authorization form.
Prescription medications must be delivered by the parent/guardian to the school nurse in the original prescription bottle. (Sending prescription medication to school with the student is not permitted.) Prescription bottles must match the medication order signed by the provider.
