Special Accommodations for Acute Medical Concerns
If your child has an acute medical condition that requires temporary accommodations, e.g., activity restriction after a broken bone, a signed note by the student's medical provider must be filed with the nurse. Such notes must outline the student's medical diagnosis, the specific restriction(s), duration of the restriction(s), and date of follow-up. Notes can be hand delivered to the front office, faxed to the school, or emailed directly to the nurse. Depending on the diagnosis, additional care plan paperwork may be required.
