The following steps must be completed before you can be matched with an MS 1 student. All preceptors must have a faculty title:
Complete the New Preceptor Intake Form to provide us with your site details and contact information.
Apply for faculty status by clicking HERE; please use the "Login with your email" option. Holding a faculty title is an LCME requirement to participate in the CCE course and we will need your application to at least be submitted before students can begin to come to your site.
You will need the following materials for your application:
Current CV (updated within last year and reflects current/ anticipated AZ employer) and a Letter of Recommendation (Please use Dr. Luba's information provided below for your letter of recommendation)
Dr. Luba's information for your letter of recommendation (We recommend choosing Dr. Luba to compose your LOR to save you time)

Complete the 30-minute online orientation at the end of the application before you submit.

Add Contact: dezamora@arizona.edu- We must have an affiliation agreement in place at your site before students can start their CCE sessions. Danielle Halbert, the Registrar, will reach out to either you or your practice manager with instructions on how to complete this agreement. Please keep an eye out for her emails and do not delete.
