Textbook Request Forms

Principals or their designees may request additional Savvas, Ready Math, Amplify ELA and Reveal Math workbooks, and Mystery Science kits through this form. 

To request teacher kits, teacher editions, Odell High School Literacy Program, and Activate OpenSciEd, please email Yvette Dodge.

High School principals may request HQIM for AP, CTE, and HB 171 and Academies aligned courses through this form.
