UPDATE - The request form is open. We now only have one request form, for all consumable programs! Use this form to Request Additional Consumables for SY 2025-2026. We have done our best to send you the instructional materials you need for SY 2025-2026. Due to enrollment fluctuations, you may need additional materials when students return.
Before requesting materials, check everything you received against the pick tickets for your school. After checking your pick tickets and locating all materials at your site, if you need additional materials, you may request them through the form (for consumables) or by emailing Yvette Dodge (for district-provided programs not listed above).
Please include in your email or be prepared to provide the following information:
1) What program you need;
2) How many books you need;
3) An explanation for why you need materials - for example, a comparison between the number of workbooks you received and the number of students you anticipate.
Note: We still have some Fundations materials in the warehouse, but will no longer order additional Fundations materials, moving forward. We will distribute the materials we have, upon principal request, in the order we receive requests. To make a request, email Yvette Dodge (dodge_y@aps.edu)
