About the Role
The Care Liaison plays a central role in coordinating care services, building relationships with referral sources, and ensuring top-notch client and caregiver satisfaction. This is a dual office-field position with scheduled hours of 10 AM–2 PM Monday–Friday in the office (minimum 20 hours/week), with the flexibility to support the field based on need (up to 40+ hours per week). Occasional evening, overnight or weekend assignments and on-call backup may be required based on client and staffing needs.
Key Responsibilities
Build and maintain strong relationships with clients, caregivers and referral partners through proactive communication, timely updates, and in-person visits or outreach activities
Serve as a central point of contact to ensure a smooth, high-quality care experience
Coordinate caregiver-client matching and support onboarding (training provided)
Handle calls, leads, and documentation in our CRM
Manage intake and onboarding tasks in our CRM system and ensure timely follow-up
Participate in outreach, check-ins, and relationship-building with community partners
Conduct touchpoints and gather feedback to monitor care quality
Provide personal care or companion services in the field as needed to support care continuity or assist when backup is needed
Ensure services are consistently delivered according to the service plan
Training will be provided upon hire.
Personal Care Responsibilities
This position will require providing direct personal care services in the field. The Care Liaison must be able to perform standard personal care aide (PCA) duties when needed, which include but are not limited to:
Assisting with Activities of Daily Living (ADLs): bathing, dressing, toileting, grooming, mobility support
Providing companionship and emotional support
Medication reminders
Light housekeeping and meal prep
Transportation and errand support
Observing and reporting changes in client condition
Supporting specialty care needs such as memory care, hospice, or chronic conditions
Minimum Requirements
2+ years of caregiving experience with ADLs such as bathing, toileting, dressing, grooming, and mobility
3+ years in customer service, care coordination, or office work
Reliable transportation (travel in York/Lancaster/Chester County may be required)
Comfortable with flexible hours and occasional evenings/weekends
Excellent communication and multitasking skills
Proficient with web tools, applications, and documentation systems
Must pass background screening and complete required trainings
Must be at least 18 years old
Bonus Opportunity
