About the position
Are you a dedicated and compassionate Caregiver with a passion for providing exceptional care to disabled and aging adults? A New Leaf Home Care is seeking a highly motivated PRN Caregiver I (Caregiver) to join our team. As a PRN Caregiver, you will play a crucial role in ensuring the highest quality of care for our clients.
The PRN Caregiver plays a pivotal role in delivering and documenting home care services aligned with the authorized care plan tailored to each client's unique needs. Home care companion & helper duties may include light housekeeping, laundry, meal prep, transportation, companionship, and advice on such things as nutrition, cleanliness and household activities. The Caregiver operates within the Company's guidelines and adheres to LTC insurance/Medicaid Case orders, ensuring service provision as directed by the contract and/or program, under the supervision of a Care Manager and/or Supervisory RN. Additionally, the Caregiver strictly adheres to State and Local Laws, Patient Bill of Rights and written agreements. Performs other duties as assigned/necessary.

Minimum Requirements:
Experience with caring for elderly and disabled adults
Proficiency in utilizing various applications and web tools commonly used in professional environments, particularly those related to document management, communication, and data entry
Clean background record and ability to pass a drug scree
Possesses and maintains good physical stamina and mental health and can pass a pre-employment health assessment and tuberculosis screening
Ability to function with minimal supervision
Reliable means of transportation
Travel: York/Lancaster County travel may be required for business purposes. If so, the employee must have a valid driver’s license issued by the state in which they work and a satisfactory driving record.
Successful completion of A New Leaf Home Care Orientation and Compliance training, and all other assigned trainings as applicable required
Successful completion of A New Leaf Home Care's Competency Program
Proficiency in the English language required
Employees: Must maintain all state and federal compliance requirements for employment
Must be at least 18 years old
Preferred:
Long Term Care Experience
Responsibilities:
Follow care plan
Assist basic client transfers providing the client has been assessed as being capable of ambulating without assistance; and/or providing another trained caregiver (including family) is involved in the transfer
Observe the client's functioning and report to supervisor
Provide companionship, friendship and emotional support
Medication reminding
Talk, listen, share experiences, play games/cards, read to client, etc.
Help keep clients in contact with family, friends, and the outside world
Provide transportation to medical appointments, grocery store and errands, which may include handling the client's money, in accordance with care plan and under the observation of supervisor
Accompany clients to recreational and/or social events
Assist with plans for visits and outings
Write or type letters/correspondence
Organize and read mail
Teach/perform mean planning and preparation
Perform light housekeeping
Assist with pet and plant care
Submit and maintain accurate and timely documentation
Communicate with supervisor and co-workers
Ensure services are consistently delivered according to the service plan and document that all client needs are being met
Be proactive in identifying any changes in the participant's service needs and promptly report them
Attend orientation, in-service training sessions and staff meetings
Physical & Mental Demands:
Must be able to complete all physical & mental demands of the job, which may include, but not limited to the following:
Good physical and mental health
Carry supplies
Physical ability to stand, walk, use hands and fingers, reach, stoop, kneel, crouch, talk, hear and see
Physical and mental ability to drive a vehicle
This is a PRN (as needed) position, allowing for flexibility in schedule based on your availability. Must complete relevant background checks, drug tests and random drug tests at and throughout hire. Compensation will be negotiated based on our mutually agreed rate, reflecting the value of the services you will be providing. While this opportunity may start with fewer hours initially, it is expected to grow as our client base expands.
