About the position
Are you a dedicated and compassionate RN with a passion for providing exceptional care to disabled and aging adults? A New Leaf Home Care is seeking a highly motivated PRN Assessment Nurse to join our team. As a PRN Assessment Nurse, you will play a crucial role in evaluating the competency of our aides, providing essential training, and ensuring the highest quality of care for our clients.
The PRN Assessment Nurse plays a pivotal role in delivering and documenting home care services aligned with the authorized care plan tailored to each client's unique needs. The Registered Nurse operates within the Company's guidelines and adheres to LTC insurance/Medicaid Case orders, ensuring service provision as directed by the contract and/or program, under the supervision of an Admin. Additionally, the RN strictly adheres to Nurse Practice Act, Standards of Nursing Practice, Standards of Patient Care, Standards of Accrediting Body, State and Local Laws, Patient Bill of Rights and written agreements. Performs other duties as assigned/necessary.

Minimum Requirements:
Currently licensed as a Registered Nurse in specific state required
Complies with accepted professional standards and practice
One year prior professional nursing experience, preferably with home care experience
Current CPR Certification
Possesses and maintains good physical stamina and mental health and can pass a pre-employment health assessment and tuberculosis screening
Fundamental knowledge of the basic principles of nursing and the ability to apply these principles in a home care setting
Has excellent writing and charting skills
Proficiency in utilizing various applications and web tools commonly used in professional environments, particularly those related to document management, communication, and data entry
Ability to accurately assess patient health care status and psycho social needs
Ability to establish a Plan of Care which will assist the patient to reach desirable health status
Ability to function with minimal supervision
Reliable means of transportation
Travel: York/Lancaster County travel may be required for business purposes. If so, the employee must have a valid driver’s license issued by the state in which they work and a satisfactory driving record.
Successful completion of A New Leaf Home Care Orientation and Compliance training, and all other assigned trainings as applicable required
Successful completion of A New Leaf Home Care's Competency Program
Proficiency in the English language required
Must maintain all state and federal compliance requirements for licensure and employment
Preferred:
Long Term Care Experience
Medicaid Experience
Working Knowledge in a supervisor capacity; in any career
Strong Leadership Abilities in long term care or like environment
Responsibilities:
Evaluate the competency of our aides, assessing their ability to carry out assigned duties and their rapport with the clients
Documents all physician collaboration/coordination of care in the medical record and communicates this information as needed
Conduct training sessions for aides through individual instruction, group meetings, or workshops, covering a wide range of topics, including fire safety, first aid, ethics, and disability orientation
Complete the Competency Test, incorporating elements of our services and essential topics related to care provision
Visit the homes of clients prior to the start of home care services and create a task list for caregivers based on the authorized service plan
Provide documentation for advanced directives acknowledgement and inform clients of their right to complain and the complaint registration process
Perform supervisory visits to the clients' place of residence within 30 days of initiating home care services and at least once every four months and document as necessary
Submit and maintain accurate and timely documentation
Ensure services are consistently delivered according to the service plan and document that all client needs are being met
Be proactive in identifying any changes in the clients' service needs and promptly report them
Be reasonably accessible by phone while services are being provided to clients
Physical & Mental Demands:
Must be able to complete all physical & mental demands of the job, which may include, but not limited to the following:
Lift and transfer clients and carry supplies
This is a PRN (as needed) position, allowing for flexibility in schedule based on your availability. Must complete relevant background checks, drug tests and random drug tests at and throughout hire. Compensation will be negotiated based on our mutually agreed rate, reflecting the value of the services you will be providing. While this opportunity may start with fewer hours initially, it is expected to grow as our client base expands.
