REQUIRED DOCUMENTS

• Verification of the child's date of birth. This can be a copy of the birth certificate or a copy of the page in the applicant’s passport on which the date of birth is recorded
• Recent 3” x 4” headshot photograph of child (under 20kb in file size)
• Copy of the applicant's most recent immunization records in one PDF to include:
  • Hepatitis B (HepB)
  • Diphtheria, Tetanus, Pertussis (DtaP)
  • H. Influenzae Type b (Hib)
  • Polio
  • Measles, Mumps, Rubella (MMR)
  • Varicella (Chicken Pox)
  • Yellow Fever
  • Typhoid
• Copy of parent’s/guardian’s U.S. passport for proof of American citizenship if applicant is to be considered for enrollment in category 3 and 6
• Copy of parent’s STR Visa/Cerpac for priority categories 3 and 4
• ALL school reports for the last two years, including transcripts for grades 9 to 11
• ALL standardized test results, to include any IEPs and psychoeducational evaluations
• AISL confidential school report to be completed by your child’s school
