You will be greeted with the landing page shown below .
You will be interested in the three areas highlighted in red.
Once at the search page you can:
Enter a facility name, if you know it, and all pertinent information will be shown for the facility.
If you don't have a name and want a listing of facilities in a certain city enter the city name.
If you want a listing of facilities in a certain zip code just enter the zip code.
