TRANSCRIPT RELEASE FORM 

for current AHS students

TWO STEP PROCESS to submit signed Transcript Release Form:

PARENTS/GUARDIANS: log into your PowerSchool Parent Portal account to sign your senior's transcript release form. "Senior Transcript Release Form" in the "Forms" sections. This gives Annapolis High School permission to send your student's transcript to any colleges and universities to which they are applying. **Please note that a paper copy may be obtained through the Counseling Center**

**BOTH Student & Parent/Guardian have to sign under their PowerSchool Portal account & Submit!
