College Visit Absence Form

This form is to be completed by the student, parent (signature required) and teachers for the day(s) a student is absence to visit a college.  Students are allowed 2 excused absences their junior year for college visits and another 2 excused absences their senior year for college visits.

Graduation "Credit Check" Sheet

I complete one of these for each junior and senior every Fall and Spring to see that they are "on track" to graduate.  All students in grades 9-11 receive this form, completed by me, during our Spring scheduling meetings to help them best plan for the upcoming academic year.
