• Please bring one to the clinics you attend, and take PRIMARY responsibility for using this document.  You can discuss with your preceptor at the BEGINNING of clinic what your education goals are for the clinic, and then wrap-up with how it went by the end of clinic.  Either the preceptor OR YOU can complete the document.   

• Your primary preceptor will need these (paper or electronic) PRIOR to your exit interview, to inform their rotation ITER completion.

• Please upload these in a batch (one PDF document) to your eportfolio at the end of each rotation, along with a PDF of your ITER.  These will be reviewed at your 6-month meetings with the program director (or sooner as needed).
