I have also shared with you a couple of forms, the first is one I use as standard homework and for flipped learning. The second is one we have used in maths as a department to collect question by question scores on test for analysis.
One of the great things about these is you can just make a copy for yourself and that will belong to you to edit, you do not need to start from scratch each time you make a Google form.
