Each year, student athletes and their parents must complete the required UIL forms. We have made it as easy as possible providing each form online. RankOneSport.com is the system that LISD utilizes for parents to submit the required UIL Athletic Paperwork. In mid-April of each year the online forms section will become active for the following school year's forms to be completed.

In addition to UIL forms, student athletes must turn in the UIL pre-participation physical form for 2024-2025 with three signatures including the student, parent and licensed physician.
