The Wilson School District has established a medication policy to allow a student to take or be given medications at school. Medications are defined as any prescribed or over the counter medications (including inhalers and Epi-Pens).
PROCEDURES:
Complete an Medication Authorization Form, signed by both the prescribing physician and the parent. (This pertains to over the counter medications, as well as, prescribed medications.) Forms can be found in the health room or downloaded by clicking the Access District Health Forms link.
All medications must be hand delivered by the parent/guardian to the school nurse.
All medication, including over the counter products, must be in its original container and labeled with the student's name, drug name, dosage, and time to be given at school. NOTE: Upon request, most pharmacies will provide two labeled containers for a prescription so that the medication can be brought to school in the properly labeled container.
The medication is counted by the school nurse upon receipt and kept in a secure, locked area in the nurse's office.
Whenever there is a change in medication dosage or direction, the school nurse must be notified in writing by the parent, accompanied by a physician's note.
At the end of the school year, a parent/guardian must pick up any unused medication. Any unclaimed medication left in the nurse's office at the end of the school year will be destroyed.
