Caption Sheets must include all of the following:
Title of Assignment
Student Name
Appropriate standard(s) – copy the standard that best fits the assignment – this should be a content standard for the individual subject and/or an English standard.
SLO – write out the SLO that best fits the assignment
Description of the assignment
Your Reflection. These should be your words. Don't have a computer reflect for you. Ew.
Purpose of the Assignment: Indicate the year and class in which you completed the assignment or project. Explain what the paper or project required. (Be sure to include enough details so your readers can easily understand not only the assignment but also the steps taken to complete it.)
Reflect on the assignment you have selected for your class with 3-5 quality sentences reflecting on your work and your process. Some questions to ask yourself / items to consider as you reflect might be:
*Show how the assignment meets the particular SLO and the standard being addressed.
*Describe how it connects to an element of the SAUSD Graduate Profile.
*Reflect on your thinking, learning, and work.
*What are you most proud of?
*Analyze what you learned by completing this particular assignment.
*Explain how this piece of evidence connects to your future goals.
*Evaluate the significance of the piece. (Why is this piece or project important enough to include in your portfolio?)
*Where did you encounter difficulty with the assignment, and how did you overcome the obstacles?
*If you could do this assignment over, what would you do differently?
