Make a Referral

1. Guidelines for Teacher/Staff submitting SAP referrals.

• Please start by talking with the student's guidance counselor.

  1. All referrals are confidential.

  2. No disciplinary action happens because of referral.

• If you think someone is in danger of harming themselves or others, you should always do at least one of the following before filling out this form:

  1. Remember that referrals may not be received outside normal business hours.

  2. Contact a guidance counselor, teacher, or principal.

  3. Contact a parent or guardian or other adult.

  4. Contact Safe2Say.

  5. Call 911 if you think they are in immediate danger.

2. Guidelines for students or community members submitting SAP referrals:

• Anonymous referrals are allowed. No login required.

  1. You have the option to leave contact information on the form.

• All of the guidelines above still apply.

• Please only make referrals in good faith. No fake or "joke" referrals.
