Important Forms

In order to participate in baseball, players must be registered and proper forms must be completed in the Genesis Parent Portal.

ALSO **Physicals**: Athletes are to check in with the Nurse either BEFORE or AFTER school to see what requirements are needed in order to be eligible to begin active participation.

*IF an athlete does not fulfill the necessary requirements then that individual will not be allowed to try out for that sport.

Transportation Form

We require that all our baseball players travel home on the team bus together.

But if there is an emergency and the parent must transport his son home from a game, then you must fill out the form provided below and submit it to the head coach prior to the event. Thank you for your understanding.
