Medication for children enrolled that may need to be administered at Kids Klub needs to be turned in with either the Assistant Supervisor or the Supervisor. We will also need the applicable medication forms on file. We do not have access to any medication left in the nurse’s office, so it is important that you give Kids Klub a supply. Medications must be current, as we cannot administer an expired medication. Links to medication forms are linked below. Please complete any forms associated with your child's medical needs, if applicable, and turn in to your site supervisors.
Medications must be in their original container labeled with the child’s full name and date brought to Kids Klub.
Medication will be dispensed per doctor’s instructions or according to the label.
Expired medication will not be administered.
Notice of and Consent for Health-Related Services During Kids Klub Afterschool
In accordance with law, the District must provide parents with written notice of each school-based health-related service offered at the campus their child attends. These routine services promote student safety, wellness, and readiness to learn. The services may be provided by Kids Klub employees. A parent has the right to withhold consent for or decline any health-related service:
Kids Klub staff will communicate with parents or guardians during or after an illness or injury. The account owner is the primary point of contact. In the event of an emergency, if our staff are unable to reach a parent or guardian, they will attempt to reach the emergency contacts listed in your child's contract. Please note that Kids Klub staff will only have access to information in our registration system for the current year or summer program. Information provided to your school campus nurse, teachers, staff, or administration may not be accessible to Kids Klub staff.
Routine Health-Related Services Provided at Your Child’s Campus:
First aid, injury evaluation and treatment
Administration of over-the-counter medications in accordance with law
Administration of prescription medications in accordance with law

Heat illness prevention and injury support
Any additional health-related services that the campus may believe are necessary and appropriate will necessitate separate written consent and parental involvement, unless during an emergency or as allowed by law.
All District employees:
Shall report alleged abuse or neglect of a student as required by law; and
May verbally inquire about a student’s daily well-being without parental consent.
CSISD Community Education programs do not offer formal counseling services, but will report any safety or well-being concerns to parents/guardians, program directors, or school counselors or administration, as appropriate.
Parents must designate whether they consent or do not consent to their child receiving routine, school-based health services in their registration.
