Kids Klub '26-'27 Registration opens May 4 @ 10am
By enrolling your child in Community Education Programs, it is understood that you will follow and support the policies stated below and included in any handbooks associated with these programs.
I give my permission for
the staff of the Community Education programs to authorize medical treatment and to transport my child in the event of a medical emergency. I understand that every effort will be made by staff to notify me should such an emergency arise.
Community Education programs to use pictures and videos for CSISD publication purposes.
my child(ren) to be transported in CSISD vehicles, to attend field trips and participate in water activities if scheduled as a part of the program offerings.
I understand the Parent Handbook for Kids Klub Afterschool can be found on the Kids Klub website at kidsklub.csisd.org. I have been given the information regarding the website address and how to access the Parent Handbook. I will review the handbook to become familiar with the policies and procedures of the Community Education program of which I have enrolled my child/children. I also understand that I am accountable for all of the information within.
In addition, I understand that tuition will not be refunded if my child is withdrawn from the program because of disciplinary reasons.
I hereby release the College Station Independent School District and their representatives from any and all liability, loss, damage, costs, claims, and/or causes of action, including but not limited to all bodily injuries, and property damage arising from my child/children participating in Community Education programs.
Only medications that are required to enable a participant to stay on site may be given at the site. Three times a day medication should be given before school, after school, and at bedtime. If necessary, medication can be given at the site under the following conditions: All medications must be brought to school by a parent/guardian and signed for with the campus Supervisors or Site Administrative Assistant. Non-Prescription (over-the-counter) medications must be in the original (unopened) container with the label intact and non-expired (includes cough drops). Community Education staff do not have access to medications provided to the school or school nurse.
Prescription medications must be in a container with a pharmacy label.
The initial dose of any medication new to the student must be given at home to observe for adverse reactions before it can be administered at school.
Any change in dosing will require a written order with physician signature.
Self-carry/self-administered emergency medications require an emergency plan and parental permission.
By completing your registration for CSISD Community Education programs, you agree to the following statements. Agreement to these statements is required in order to participate in Community Education Programs.
I have read and understand CSISD's medication policy.
I hereby authorize the school authorities to act on our behalf in case of emergency when a parent or guardian cannot be reached. I understand that cost of emergency services remains the responsibility of the parent or guardian and will not be assumed by the Superintendent, the designee, the Board of Trustees or the College Station Independent School District.
I understand the school may disclose the above information to those within the school district that have a need to know for educational purposes.
I understand that the Board, the School District, and its employees shall be immune from civil liability due to allergic reactions or other injuries resulting from the administration of medication to my child, provided such administration conforms to the requirement of this policy.
I understand that the Nursing Practice Act and Texas Administrative Code 5217.11 (D)(vi) compels the RN or LVN to contact other healthcare team members, including the prescribing physician, concerning significant events regarding the patient's status.
