Nomination Forms

Use the following forms to nominate your student or yourself for E.L.P. You may print, fill out, and submit the form to your school's TAG teacher, or you may make a copy of the document and submit it electronically. Identification decisions for incoming 7th-12th graders are made in May of the school year prior. Identification decisions for 5th-6th graders, as well as for students new to the district, are typically made in August or early September, approximately 2-3 weeks after the start of the school year.
