The Evaluation Checksheet
Contained in your Eval folder, this spreadsheet will let you see what the Evaluation Committee has received and what might still be missing from your portfolio and evaluation materials for the year. Please let Jim Heath or June Mullin know if anything is incorrect so that the checklist can be updated before the Evaluation Committee meets. For information on what is required for portfolios, review this information.
The main headings are:
Identity and basic information: Who is being evaluated, whether a portfolio is due, whether any materials have been received.
Evaluation summary: Includes the items that will be reviewed for everyone each year.
Action plan: If an action plan was recommended the previous evaluation cycle.
Faculty Input Forms: Whether or not FIFs were received, by semester or year.
Student Evaluations: What Student Evaluations ratings were for each class taught.
Grade distributions: “Unusual” grade distributions for any classes. Unusual” grade distributions means >60% As and >50% Ws or Fs – please comment in FIFs about this.
Assessment Compliance: Was this done when required?
Professional Development Requirements: Completed or incomplete.

Some definitions used:
EX = Excellent
VG = Very Good
G = Good or Fair
Unacceptable
DNT = Did not teach
In progress – information is present, but may not be complete
