Emergency Medical Forms are filled out online through Info Snap. It is important that these forms be filled out as soon as possible. Your child MUST have this form completed prior to any field trips. Your child MAY NOT attend any field trips until this form is completely filled out and submitted. If you have questions accessing your child's Info Snap account, please contact your building secretary. 

Parents that may have a child with any medical concerns, physical handicap, asthma, diabetes, heart or seizure disorder, food or environmental allergy, etc. need to notify the school nurse to discuss your child’s health care needs.  It is very important that the required paperwork be filled out and returned to the nurse as soon as possible so we can assist your child with any health care needs he/she may have or that might arise during the school year.  Please keep all emergency contact information and health needs current throughout the school year. 

*Please keep the nursing staff informed about the conditions that your child may have and any changes in your student's health condition or plan of care. This is a great way for us to assist with your child's health and any follow-ups that may be needed at school.
