Allergies

If your child has been diagnosed with a severe allergy and will need to keep an emergency medication at school, please have your doctor fill out the appropriate medication form; either self-carry (if age appropriate) or permission for medication at school prescription form, and an allergy action plan. If your child needs a substitution for the cafeteria (ex: substituting a milk for a juice) due to an allergy, we will require a doctor's note. Please inform your school nurse of any questions or concerns.
