All past students requesting transcripts for the purpose of attending an Ontario College or Ontario University should be using the eTMS system. Through OCAS and OUAC application process which is initiated by the student, the Electronic Transcript Management System (eTMS) sends secondary school transcript information for past students to Colleges and Universities electronically. Do not complete the Sacred Heart CHS Transcript Request form unless for the following purposes:
· Employment
· Post Secondary Education OUTSIDE of ONTARIO
· Scholarship
· Summer School
· Personal / Other
To obtain an official transcript, please complete the transcript form.
There shall be no charge for a transcript for current students (2023-24) or for one (1) year following graduation.
For all other students, there is a $25.00 fee to generate a copy for the first copy and $5.00 for each additional copy. An additional fee for postage will be added if applicable. Payment can be made in person in the form of cash or cheque.

Former student of the YCDSB applying to an Ontario University
Transcript requests for College or University can be completed online through OCAS (for the colleges) or OUAC (for the universities). To use this service, the student must:
Be a past student of the YCDSB
Apply to a minimum of one Ontario College or University
Pay the application and transcript fees

Current YCDSB students who are applying to Ontario colleges do not need to request a transcript. Transcript information will be forwarded from OCAS to the post secondary institution.
