The General Health & Emergency Contact Form is REQUIRED and will be a virtual Google Form. Due by Friday, May 8th of 2026.
The form will be emailed to parents in March.
The Self-Administered Medication Form will be a paper form. Must be signed by a guardian and licensed physician, and is due by Friday, May 8th of 2026.
The paper self-medication form will be given out during homeroom during the last week of March.
This form is only necessary for 8th graders bringing over-the-counter and/or prescription medication on the trip. See Forms to Return tab for extra copies.
Medications will be self-administered (no nurse on the trip)
Only medications listed on their Self-Administration Health Form can be taken while on the trip
Both prescription and non-prescription medications must be signed off by a physician and guardian (i.e.- epipen, allergy medications, etc.)
Tylenol, Ibuprofen, Tums and Cough Drops do not need to be signed off on but still need to be packed and self-administered by the student. These medications can be self administered with parent permission and with previous permission given by the parent noted in Genesis.
Medications (including epipens) must be in the original containers and brought by the students; chaperones will not have medicine to give.
Students cannot share medications with other students under any circumstances
A ‘nurse’s suite’ will be available in the hotel for any student that falls ill overnight; students’ should tell a chaperone if they are not feeling well
Nearby medical services are accessible in case of an emergency
Notify Mrs. Yadoff and Ms. Silvestri via email if there are any undocumented changes to your child’s medication needs between the time the form was submitted and the start of the trip.
