*EMS Permission Slip:
This form is required for all students going on the trip.
Completed and returned to homeroom teacher by Friday, 5 December 2026 with the $150 non-refundable deposit.
Instructions to print at home:
Click on the gray icon at the top right of the document to open it in a new tab.
Once the document is opened, select File > Print
Self-Administered Medication Form (PAPER ONLY):
Must be submitted by Friday, May 8th, 2026.
This form is required ONLY if your child has to take prescription OR nonprescription medication during the school trip.
Must have a parent/guardian AND physician signature.
Medications will be self-administered (no nurse on the trip).
Students are NOT permitted to share medications with other students. Doing so will result in removal from the trip.
Please send Ms. Silvestri an email documenting any changes between the form's due date and the trip start.
