Travel Release Form

Please email Travel Release to your COACH. See email addresses above.

All physicals dated on or after 7/1/2024 will need to be on the latest version of the state approved physical form. The latest version of the physical form has been available on the athletic webpage (Form #3) since 5/1/2014. 

The cardiac module is a new cardiac screening requirement for all New Jersey high school athletes. This new component must be completed on all physicals dated on or after 7/1/2015. Doctors that possess a cardiac module certificate may sign off on this section of the physical form. Please be certain to check that your doctor has the proper training to complete the cardiac screening portion of the physical form. The cardiac screening sign-off is located at the very bottom of page 3 of the physical.

Season start date information

Summer period:

6/20-8/31 - all sports may workout with hs coaches.  

7/29/23 - 8/6/23 -  non-contact period.  


FALL SPORTS:

8/16/23 - girls tennis

8/23/23 - Fiel Hockey

8/26/22 - all other fall sports


WINTER SPORTS: 

11/20/23 for hockey/swim/bowling & 11/27/23 for all sports.


SPRING SPORTS: 

3/11/24 for tennis & 3/14/24 for all others.
