In conjunction with your hard copy physical form and health history update form, all student-athletes and their parents must complete their registration with FamilyID. This must be completed for each team that your child wishes to participate on. Please click the link below to begin registration.

How do I know if I need a full physical form submitted or just a health history update/covid form?
***The state of NJ is requiring both forms because of the Covid questions added to the HHQ, regardless of the date of your child's last physical.
PHYSICAL INFORMATION - VERY IMPORTANT:
Physical forms and health history update forms must be submitted to the school nurse no later than 7/15/24 to guarantee clearance by the start of the spring season. You must use the state forms, which can be found on the athletic website, and the cardiac screening module must be signed off by your physician. A physical is good for 365 days. Below are examples of when your physical expires and what that means.
Physical Expiration Examples:
Fall sports begin on 8/19 (on example).
Example 1 - Physical expires prior to the start date - you would need to submit the full physical form prior to the start of the fall sport (8/19 to guarantee clearance).
Example 2 - Physical expires after the start date, but during the fall season - The physical is good for the duration of the fall season. The student needs to submit a health history update form. This student would need a new physical prior to competing in a fall or winter sport.
Example 3 - Physical expires after the fall season, sometime in the winter or the spring. This student would just need to submit a health history update form.
