Please send in a copy of your child's most recent physical and updated immunization records so I can keep your child's medical records current.

It is very important for the school nurse to know when your child has been diagnosed with any medical conditions.   Such as:  Asthma, Allergies, Diabetes, Heart Conditions & Seizures.

Please notify me of any changes in your child's health due to illness or injury. 

Emergency contact information is very important.  Please keep this information current in Genesis.
