Medication

 Please send in the Authorization by Parent and Healthcare Provider for Administration of Medication In School form signed by MD and parents for Nurse to give necessary medication during the school day.

Medication prescribed by MD should be brought into school by parents in the original packaging from the pharmacy properly labeled for the child.

Authorizations are effective for one school year and must be renewed annually.  

Children should not carry any medication.  Medication should be dropped off to Nurse by parent and picked up at the end of the school year by a parent.
