Diabetes

 If your child has diabetes, please send in the Diabetes Medical Management Plan signed by MD and parents.

Medication prescribed by MD, glucometer, strips and other supplies should be brought into school by parents in the original packaging from the pharmacy properly labeled for the child.  Emergency snacks and juice kits should be sent to Nurse & classroom.

 Orders are effective for one school year and must be renewed annually.  

Children should not carry any medication or sharps.  Parents please drop off to Nurse and pick up at the end of the school year.
