Allergy

 Please send in Allergy Emergency Treatment Plan signed by MD and parents.

Medication prescribed by MD should be brought into school by parents in the original packaging from the pharmacy properly labeled for the child.

Epinephrine is prescribed for 2 injections.  Please send in both doses in package.  

Authorizations are effective for one school year and must be renewed annually.  

Children should not carry any medication.  Medication should be dropped off to Nurse by parent and picked up at the end of the school year by a parent.
