Immunizations

The State of Maine immunization requirements for students to be allowed to attend school are as follows, unless a letter of exemption has been signed. Please note that per the state law, your child will not be able to attend school if we do not have an up-to-date immunization record or an exemption letter on file annually.

Pre-K: 4 DTap/DTP

3 Polio

1 MMR

1 Varicella

K-6: 5 DTap/DTP (4 if the 4^th dose is given on or after 4^th birthday)

4 Polio (3 if the 3^rd dose is given on or after 4^th birthday)

2 MMR

1 Varicella

Should you choose not to have your child further immunized then an Exemption Letter must be completed and signed by both parents. If you are divorced/separated but share parental custody, we will need both parents to sign an exemption letter.
