Chronic Conditions

Good management and clear communication of your child’s health between us and your health care provider(s) are important for success in our school.

If your student has a chronic medical condition, they will need to have an action plan on file in the nurse's office. You can click on one of the buttons below to download a copy of an action plan.

If you do not see an action plan listed here that is applicable to your student's condition, please contact me.

All action plans will need to be signed by the guardian and the doctor.

Allergies

Allergy Action Plan or one that your doctor uses.

Food allergy is a serious medical condition affecting up to 15 million people in the United States, including 1 in 13 children. A food allergy results when the immune system mistakenly targets a harmless food protein – an allergen – as a threat and attacks it. Food allergies can be life threatening. The risk of accidental exposure to foods can be reduced in the school setting if schools work with students, parents, and physicians to minimize risks and provide a safe educational environment for food-allergic students. We want to work closely with you to provide allergen-free meals to students with a documented medical need.
This pertains to ANY ALLERGY (like bees)

In order to keep your child safe at school, we are asking for a written physician's note. Following their Allergy Action Plan is important to all of us.
If your child has an Allergy Action plan, please bring a copy or fax it to the office.
If your child does not have a plan, please talk to your provider about developing one or a note that documents signs and symptoms for us to look out for if exposed to known allergen(s).
If your child has a history of food allergies but does not need a plan, please let us know too. We will update this information.

EpiPens may be carried in a student backpack IF:
- They have an allergy action plan complete with the self-carry agreement signed by the doctor, guardian, and student.
- The EpiPen is individually labeled with a pharmacy label.

Asthma

Asthma Action Plan

A written physician's order is needed to help keep your child safe at school. Following the Asthma Action Plan is important to all of us.

If your child has an Asthma Action Plan, please send a copy or fax it to the office.
If your child does not have a plan, please talk to your provider about developing one that can be shared with the school staff.
If your child has a history of asthma but does not need an inhaler in school, please let us know too. We will update this information.
Inhalers may be carried in a student back pack IF:
- They have an asthma action plan signed by the doctor and guardian on file with the nurse.
- The inhaler is in the original box with pharmacy label OR the inhaler has a pharmacy label on it.
