First question... What does EEA stand for? It stands for Equivalent Educational Activity.
Next question... Why would I want to do an EEA? Most likely, you will need to write one before you do a book study.
Final question... What will I need to know before I fill out the EEA form?
Book title
Collaboration: Who will be participating, Who will be tracking attendance
Timeline: starting month, finishing month, pacing of the chapters (by week or by month), Calculation of hours and CEUs
Here is an example:
December 2018-May 2019 See attached pacing. 1 hour per chapter for reading and providing responses for discussion questions. 30 minutes per discussion questions that will guide implementation into classrooms. Total 15 hours = 1.5 CEUs
(Calculation: 12 chapters, 1 hour per chapter, 6 meeting dates, 30 minutes per meeting, 12+3=15 hours)

Here is where you insert ALL of that information we collected at the top of this page. Your title can be the Title of the book followed by the words "book study.
Then, you will click the box next to the doc you uploaded for your pacing. Finally, you will click Submit!
Once you are finished with your EEA, you will need to submit for CEUs. Please refer to that tab for directions. (When you do fill it out, you will need to include the pacing of the EEA, notes from book study and record of attendance.
