Community Service Opportunities

1. Take your log sheet to each place and each time you volunteer.  

2. Fill out your portion: first 4 boxes.

3. Have the event coordinator or volunteer coordinator of that agency/event complete the "agency contact" portions of the form (last 3 boxes) 

4. Turn the completed form into Ms. Martz in student services.

5. Ms. Martz will verify and enter. 

6.  **Recommendation: take a picture of your completed form before you turn it in.

*Remember each National Achiever must complete 10 volunteer service hours per semester to maintain NAS membership.  The 20 total hours must include a minimum of 14 "NAS Sponsored" hours.  This means anything listed on Hosted Groups.
