Homebound

Home Bound

"...The homebound instruction program is for students who because of a medical condition are unable to attend the regular instructional program. The homebound instruction program shall consist of three (3)  hours of instruction per week while school is in session for a period of time determined, on a case-by-case basis, by the district (Waynetn.net)...."

Process: 

1. Parent(s)/Student notifies Principal of medical condition and that they are seeking to be homebound. 

2. Parent(s)/Student has the request for homebound form(s) signed by Physician.

3. If approved, the request form shall be turned into the Supervisor of Homebound services, LaDonna Edwards.

4. The student will then be instructed with further instruction.

5. After a maximum of six (6) weeks is up, a new homebound request form must be completed. EVEN IF your physician signed off longer than six weeks of homebound.
