DC Checklist

Rally Entry

This year entry forms will be online at Blue Horse Entries. There are separate entries required for Show Jumping Rally and Dressage Rally. Please be sure your members are aware. Payment will be online with Blue Horse. Entries are due by 5/8/22.

All other forms required by USPC are available on this site.

There is a Team form to be completed by the DC. Please be sure to indicate the captain with an * next to the name.

Every competitor under 18 on a team with a competitor over 18 requires the Jr-Sr Team Form

All teams with member under 18 require a Chaperone form.

All riders over 18 require an emergency contact form.

All riders for SJ Rally require a Coach's form. This is recommended but not required for Dressage Rally.

Coggins and Rabies must be uploaded by 5/8/22. Entry to the Houghton Equestrian Center will be denied if we do not have these forms. They will NOT be accepted in hand on June 10. Horse substitution paperwork must be in by 6/8/22.
