Core Physiatry Rotations

To request time off from off-service rotations, complete this electronic form.
This form is specifically used for U of C Physiatry Residents when requesting time off for physiatry core rotations. It is the responsibility of the resident to have this form completed and returned to the program office TWO BLOCKS PRIOR to the start of the rotation.

Off-service programs will let you know if they have additional & particular steps to complete vacation requests for their program.
