There are many forms that parents and adult leaders should be aware of. These include medical forms, registration forms, permission slips, and more!
The BSA has an annual medical form that needs to be turned in to unit leadership each and every year just to participate in regular troop meetings. This would be medical form parts A and B. Part A is important because it serves as a permission slip of sorts, and part B is important because it alerts the troop leadership of allergies, asthma, and other medical conditions that can pose a risk to a scout's health during an activity.
Part C is required for any activity over three days long. This would be summer resident camp or a three day weekend trip. This form requires a doctor's signature, so please add it to your plans for your scout's annual checkup.

Parents are encouraged to volunteer where they can to help out the troop program. In addition to positions in the scoutmaster corps and troop committee, we need merit badge counselors and sometimes reserve members to help meet our adult supervision requirements. Please fill out and return the form below to one of our leaders.
Merit Badge Counselors are council-approved expects in the subject matter for an merit badge. To become one, you must provide an Merit Badge Counselor Application, YPT certification, and an adult leader application to the council.
