About the Tri-County Collaborative Value Based Care Project

As the state moves toward a new Medicaid managed care strategy, and subsequently implementing value-based care, this project supports and prepares the rural health and human services agencies in Bladen, Columbus and Robeson counties in their learning and development related to implementing the state’s transformation plan. The Tri-County Value Based Care Collaborative project aligns with the three aims of North Carolina’s Medicaid Managed Care quality strategy: (1) Better Care Delivery (2) Healthier People, Healthier Communities, and (3) Smarter Spending. ​

This project is funded by the Kate B. Reynolds Charitable Trust and Blue Cross Blue Shield Foundation.

Care Share Health Alliance- the Care Share Health Alliance, builds capacity, create networks, and partners for systems change so that communities are healthy and equitable.

Kate B. Reynolds Charitable Trust- works to improve the health and quality of life of residents in Forsyth County and around North Carolina. KBR is one of the funders of the VBC Project.
