A form completed by a travel agent when a booking is created. Areas included in booking form: the major areas that need to be included in the booking form are name, address and age of client, record of all services purchased, price of each service supplied, passport and health requirements and status,  reservation details including pnr, special requirements of the client and record of all monies paid.
