Forms for counseling intake are located online. Please direct all inquiries to our website.

We do have physical copies on hand in the file cabinet, filed under, ‘Counseling Intake Forms. If a request is made in person, have the individual begin the form while also giving them an envelope, then call Staff. Any and all other counseling program inquiries are to be made to the assigned intern, or Shelly McCallister, mmccallister@goaffirmations.org.
