Timeline/Checklist
Student Age:
Freshman/14 years- Students enter high school. Classes are integrated with academics and based on the individual needs of the student. FAPE and services are provided based on assessment, student need, and planning goals. Imbedded skills are presented though Specialized Academic Instruction (SAI) and General Education classes. Inclusion and involvement in high school clubs and activities are strongly encouraged. Attending prom, recreational activities and sporting events create great opportunities for independence and demonstrating responsibility. Students are encouraged to make plans , learn skills for IEP participation, and practice self-advocacy. If your student is not a Regional Center client, now would be a great time to apply. They provide life long services and will help with the transition process, funding, and job placement as your student ages.
Junior/16 years- Official planning for transition is completed prior to the IEP that is in effect when the student turns 16. Clearly written, measurable, attainable goals are written into the IEP in the transition section. These goals and services are based on recent assessments and discussions involving students, families, and services providers. While these goals may change, it is important to start this planning process and provided the best opportunities for students while they are completing high school classes.
Senior/18 years- Students will complete high school with their peers and receive a certificate of completion or a diploma. Students at this point who have not received a diplomat have the option to continue onto post-secondary class. In this class, students complete community outings and work specifically on post-secondary goals. This time is a period to step away from the academic classes of high school and focus on individual needs for training/education, employment, and independent living skills. Work sites based on the best available options for your student, along with staff support will be held in the local community setting.
Adult/22 years- Students typically graduate from post-secondary classes the semester they turn 22 years old. During this time, there will be an exit IEP and parents and students will be provided a Summary of Performance (SOP) review. If your student gains meaningful employment prior to this time as an adult attendance is not mandated. Your student may benefit from working in the community and already have many of the skills needed for employment. Part-time attendance with a work school balance may be best for your student. Your Regional Center case manager will be available to assist with the work/life transition at this time and after the age of 22.
