Individuals with more significant disabilities and support needs may require extensive lifelong supports. These areas include, but are not limited to financial, residential, and vocational supports. A plan needs to be in place for aging family supports and how the individual will continue to advocate, make personal choices, and continue community participation over time. In many cases-state, federal and local agencies will work through a regional center to provide support.
Tri-Counties Regional Center in Ventura County provides life-long supports for individuals based on yearly intake assessment, annual renewal, and specific disability needs. This non-profit agency is able to weave together services best fit for an individual based on best practices and with the individual’s input in mind. This is important when students leave the entitlement of a school system and enter a system of eligibility as adults. For many individuals with disabilities in Ventura County who have significant disabilities, this will be one of the first places you turn to after graduation.
Job training, placement, medical, housing, and many other individually-based supports come through the coordination of Tri-Counties Regional Center. If you or your family relocate there are currently 21 centers throughout California to keep the level of support consistent. The current recommendation is to apply as a client prior to 18. Many students have already applied and received services by high school, but if not-now during your student’s transition process would be a great place to start. If your student is still in the school system your case manager can coordinate IEP attendance and provide information on how to connect to Tri-Counties and other local agencies to aid in the transition of services and supports into adulthood.
